On 23 July 2001, City of Los Angeles Public Safety Employment Division Chief Phyllis M. Lynes mailed the following documents in response to this request:

- Los Angeles Police Department Polygraph Notification Form (76 kb PDF). This two-page form letter is used to notify LAPD applicants of the time and date of their pre-employment polygraph interrogations, where to report, guidelines to follow, and provides a strip map of Parker Center.
- LAPD polygraph policy recommendation letter from City of Los Angeles Personnel Department General Manager Margaret M. Whelan to the Los Angeles Board of Civil Service Commissioners dated 3 January 2001

Dear Ms. Lynes:
This is a request under the California Public Records Act
(California Government Code §§6250-6258).
I request that copies of the following documentation be provided
to me:
1. Any polygraph consent and/or waiver form(s) that
employees of, or applicants for employment with, the
Los Angeles Police Department (LAPD) are requested
and/or required to sign prior to administration of any
employment-related polygraph test.
2. Any Public Safety Employment Division rules,
guidelines, directives, circulars, communications, or
other instruments governing polygraph policy.
I am willing to pay copy costs arising from this request up to
$15.00 consistent with CGC §6257. Please notify me if copy costs
will exceed this amount.
In accordance with CGC §6256, I request a determination on this
request within 10 days of your receipt of it.
